Students entering Pre-K, Kindergarten, 6th grade, and 9th grade, as well as any student new to the state of Illinois, are required to submit a 𝐩𝐡𝐲𝐬𝐢𝐜𝐚𝐥 𝐞𝐱𝐚𝐦𝐢𝐧𝐚𝐭𝐢𝐨𝐧 𝐚𝐧𝐝 𝐩𝐫𝐨𝐨𝐟 𝐨𝐟 𝐫𝐞𝐪𝐮𝐢𝐫𝐞𝐝 𝐯𝐚𝐜𝐜𝐢𝐧𝐚𝐭𝐢𝐨𝐧𝐬 by the first day of school.
𝐏𝐥𝐞𝐚𝐬𝐞 𝐧𝐨𝐭𝐞: If these documents are not submitted by the first day of school, your child will not be allowed to attend until the required paperwork is turned in.
